Frost heave happens in colder temperatures and climates. This is very similar to the asphalt freeze/thaw cycle. Once the water in the soil freezes and expands, it begins to push the concrete upward. This usually happens if moisture is trapped in the soil (or foundation) beneath the concrete. A way to avoid this is by insulating the surface with styrofoam.
Spalling / Pitting
Spalling or pitting in concrete can occur from poor installation technique, or colder climates where freeze/thaw cycles take place. Complete concrete removal and replacement may be necessary if the spalling is widespread on majority of the surface.
